After each consultation, you will receive a detailed consultation letter within two weeks, and Dr. Sundaram is available to discuss complex cases by phone. She aims to be the kind of specialist colleague who makes your patients feel they are in excellent hands — and who makes your work easier, not harder.

How to Refer

Choose Your Referral Method

You may send a referral by fax to (604) 426-2720 using your clinic's standard referral form, or via Ocean Health eReferral (search "Dr. Aish Sundaram" or "Precision Eye Surgery"). Both methods are monitored daily during business hours.

Include Required Information

Please include the following in all referrals for the most efficient triage and booking:

  • Patient full name, date of birth, and MSP number
  • Referring physician / optometrist name, fax number, and CPSBC or CPSO number
  • Reason for referral (primary diagnosis or clinical question)
  • Urgency level (Urgent / Semi-Urgent / Routine)
  • Relevant ocular and medical history
  • Current medications (especially ocular drops, anticoagulants, alpha-blockers)
  • Visual acuity measurements and IOP readings, if available
  • Recent relevant investigations (OCT, visual fields, imaging) if applicable

We Triage and Book

Upon receipt of your referral, our office triages based on urgency and contacts the patient directly to schedule an appointment. Urgent referrals are prioritized for same-day or next-day review. Routine referrals are typically booked within the standard regional wait time framework.

Consultation Letter Returns to You

Dr. Sundaram sends a detailed consultation letter to the referring provider within two weeks of the patient's visit. Letters include the clinical findings, diagnosis, management plan, and any recommended follow-up. Complex cases or significant acute findings are communicated more promptly.

Referral Urgency Categories

Please clearly indicate urgency on all referrals. This allows appropriate triage and ensures patients with time-sensitive conditions are seen promptly.

Urgent
  • Acute angle closure glaucoma
  • Sudden, unexplained vision loss
  • Significant eye trauma
  • Retinal detachment symptoms (new floaters, flashing lights, visual field loss)
  • Corneal ulcer with reduced vision
  • Optic neuritis / suspected optic neuropathy
Target: Same day or within 24 hours. For true emergencies, direct patient to Emergency Department or call 911.
Semi-Urgent
  • New glaucoma diagnosis or suspected glaucoma
  • Rapidly progressing cataract
  • Suspected retinal tear (without detachment)
  • Uncontrolled intraocular pressure
  • New diabetic macular oedema
  • Significant visual field loss — new or progressing
Target: Within 1–2 weeks.
Routine
  • Stable glaucoma monitoring
  • Cataract assessment and surgery planning
  • Routine diabetic retinal examination
  • Blepharoplasty consultation (functional or cosmetic)
  • Eye disease co-management
  • Second opinion / comprehensive evaluation
Target: Within 4–8 weeks.

What to Expect After the Referral

Dr. Sundaram's office acknowledges all incoming referrals and contacts the patient directly to arrange an appointment. You do not need to follow up to confirm receipt — if there is a problem with an incomplete referral, we will contact you.

All referrals are acknowledged. Incomplete referrals are flagged and the referring office is contacted for additional information.

Patient Booking

Our office contacts the patient directly to book an appointment based on the indicated urgency level.

Consultation Letter

A detailed consultation letter is sent to the referring provider within two weeks of the patient's visit. Urgent findings are communicated promptly.

Shared Care

Dr. Sundaram supports collaborative care models. She is available to discuss complex cases by phone and welcomes ongoing communication with referring providers.

How quickly will my urgent referrals be seen?

Urgent referrals (acute angle closure, sudden vision loss, suspected retinal detachment, significant trauma) are triaged for same-day or next-day assessment where clinically possible. Please clearly mark the referral URGENT and include a brief clinical summary. For true emergencies where hours matter, direct the patient to the nearest Emergency Department or call 911 — do not wait for a specialist appointment.

What information does Dr. Sundaram need in a referral?

Does Dr. Sundaram provide shared care / co-management?

Yes. Dr. Sundaram actively supports shared care models for chronic eye conditions such as stable glaucoma, diabetic retinopathy, and age-related macular degeneration. She will indicate her recommended follow-up interval and shared care responsibilities in the consultation letter, and is available to discuss arrangements by phone.

What happens if a patient cannot attend their appointment?

If a patient cancels or does not attend, our office will contact them to rebook. If a patient fails to attend twice, a letter is sent to the referring provider advising that the patient did not attend. This ensures you are informed and can follow up directly with your patient if clinically important. We do not simply close the referral without notifying you.
